Re: Request to Appoint Mediator and Family Court Notice of ADR Forms
ORDER
Pursuant to the provisions of S.C. Const. Art. V § 4,
The Request for Appointment of Mediator form (SCADR 109) bearing a revision date of 05/2015, is approved for use in the Family Courts of the State of South Carolina. Pursuant to recent changes to Rule 4, SCRADR, the Request for Appointment of Mediator form has been created.
Pursuant to recent changes to Rule 4, SCADR, the Notice of ADR form (SCADR 102F) bearing a revision date of 05/2015, is approved for use in the Family Courts of the State of South Carolina.
These forms are available on the South Carolina Judicial Department website at www.sccourts.org/forms.
