AMENDMENTS TO FORMS 745, 426ABCD & 426E


Pursuant to the provisions of S.C. Const. Art. V, � 4,

IT IS ORDERED THAT FORMS SCCA 426ABCD and SCCA 426E are renumbered as follows:� Family Court Order, Protection from Domestic Abuse Act shall be SCCA 426, and Family Court Order Denying Relief shall be SCCA 426 Denial.

IT IS FURTHER ORDERED that Forms SCCA 745, Magistrate Court Order, Protection from Domestic Abuse Act, and SCCA 426, Family Court Order, Protection from Domestic Abuse Act, with a revision date of (04/09) are hereby revised to capture additional data to further assist in entering these protective orders into the National Crime Information Center�s (NCIC) database.

Additionally, SCCA 745 and 426 are hereby amended where necessary to provide consistency between the magistrate and family court forms, to the extent allowed by statute.� These forms may not be altered in substance except by Order of this Court and shall be available from the South Carolina Judicial Department website at www.sccourts.org/forms.� All previous versions of these forms are obsolete.� This Order is effective as of the date of its signing.�
