Re: Civil Action Coversheet SCCA/234
ORDER
Pursuant to the provisions of S.C. Const. Art. V § 4,
The Civil Action Coversheet, SCCA/234, bearing a revision date of 10/2014 is approved for use in the Circuit Courts of the State of South Carolina. The reference to the Notice of Intent Action under Nature of Action Code 220 has been revised to identify the Notice of Intent to file a Medical Malpractice Action with its new case type "NI".
The Civil Action Coversheet is required with all initial pleadings filed in the Court of Common Pleas. This coversheet should be completed, in its entirety, by the plaintiff/petitioner or their attorney when initiating an action in Common Pleas Court. This coversheet shall remain as an attachment in order to document the nature of the action that is being filed and as proof of payment of the filing fee if applicable. Form SCCA/234 (10/2014) supersedes and replaces the coversheet bearing a revision date of 06/2013.
